Research tells us that at least 1/3 of all men have high blood pressure, and women are not far behind. Much of the time this disease has no physical symptoms, but it dramatically increases your chance of dying of a heart attack and your risk of heart failure. It is the number one cause of stroke, and only second as a cause for kidney failure. Studies also show that the following are simple ways that could help control your blood pressure. For example:

Squeeze a Tennis Ball
Yes, you read that correctly. About 40 years ago, Dr. Ronald Wiley developed a new device to stop pilots from falling asleep by increasing their blood pressure and circulation to their brains. Interestingly, it was discovered that after prolonged use, blood pressure actually reduced.

You can get a similar result by squeezing a tennis ball. Just hold the ball loosely, then squeeze it as hard as you comfortably can, holding for a minimum of five seconds. Relax your whole body for a count of two, then repeat the process ten times. Rest for a five count, then do ten more repetitions with that hand. Repeat with your other hand.

Enjoy Some Dark Chocolate
Dr. Karin Ried, working with researchers from the University of Adelaide, Australia said that eating dark chocolate can "significantly, albeit modestly, reduce blood pressure for people with high blood pressure but not for people with normal blood pressure."

Another study published in the JAMA found that eating approximately 30 calories per day of dark chocolate was associated with lowered blood pressure. This was reported to occur without weight gain or other adverse effects.

Savor a Cup of Tea
Wow, I love tea! Almost a countless number of studies have shown that tea has positive effects on our blood pressure. Most of these studies have been done with green and oolong tea (a bit more robust and heartier than green, but also low in caffeine).

One study that spanned a decade and conducted by scientists at Taiwan's National Cheng Kung University, determined that drinking just 4 oz. of oolong or green teas a day can reduce the risk of high blood pressure by a 46%. When that was increased to 20 oz., researchers saw a 65% drop.

More recently, Hibiscus tea has been touted as a BP-reducer too. With no caffeine, this is a preferred choice for some. It's said that at least two cups of this mild tea per day can show results in as little as 4 weeks, and it tastes pretty good too.

Know You Are Breathing
Notice that I didn't say, "Just breathe." We are breathing all the time, so if that was all that was needed, there would be no high blood pressure. However, knowing that this is so, that is - bringing your attention to the activity of your breath - can help bring blood pressure down.

All you need to do is regularly spend 5-10 minutes sitting comfortably and allowing thoughts to drop away as you simply observe your breath. If you notice that your mind has wandered, no big deal, just come back to experiencing the inhalation and exhalation.

Rollin McCraty, Ph.D., and director of research at the HeartMath Institute in Boulder Creek, Ca, says that having a routine breathwork practice can achieve results. One three-month study of computer-company workers showed that drop could be as pronounced as ten points systolic and six points dyastolic.

Get a Massage
Ahhh, massage therapy. Recent research from Dillard University in Louisiana showed that a combination of massage therapy and diaphragmatic breathing reduced blood pressure and also anxiety in as little as one week. Another study done by the Touch Research Institute at the University of Miami School of Medicine, showed that regular massage reduced high blood pressure and depression too. As with chocolate, tea and tennis ball-squeezing, the best results were gained by regularity. (Who could resist such a scientific experiment - regular massage therapy treatments for research?) Clients I have seen who report BP improvement are receiving at least 1-2 treatments per month.

Of course, don't take all of this as a new regimen to solve your high blood pressure woes, but continue to follow your health care provider's guidance. This isn't meant to be a substitute, but a possible compliment to your current plan.

What To Expect At Your First Massage Appointment

Do you have questions like: Do I have to undress completely? Does it hurt? Is there anything I should do to prepare? What will I feel like after? What is the process?

All those questions can be answered with the simple statement - It depends upon your goals and desired outcome. Some clients expect relaxation, some expect pain relief and others expect improved athletic performance.

There are, however, some elements common to all first massage appointments and to after care instructions.

Intake and Consultation

First, you are asked to complete a client intake form. This form alerts the therapist to any special needs, areas of focus or treatment contraindication. Be as thorough as possible and be prepared to briefly discuss any conditions that will impede or improve your massage.

This is a good time to discuss the techniques used and your comfort levels; tolerance to pressure, degree of undress.

Privacy and Comfort

Following a brief consultation in the massage room, the therapist leaves you in privacy to disrobe to your comfort level. There are some massage techniques that can be performed while fully clothed if you are uncomfortable disrobing at all; Shiatsu, for example.

Lie down on the table; usually face down, with your head supported by a padded face cradle. Cover yourself with the sheets provided. You are completely covered while lying on the massage table. Only the area being treated is uncovered.

Room temperature, heating pads and body position are all adjustable. Don't hesitate to ask for what you need for your comfort.

Massage Scope and Technique

A full body massage usually includes: the face and scalp, neck, shoulders, upper and lower back, arms, hands, gluts, legs, and feet. If there are any areas to avoid, let the therapist know in during the consultation.

Most massage therapist use a combination of techniques. Two therapists trained by the same school personalize the techniques and are never exactly the same. Therefore, choosing your therapist is an intensely personal decision. Communication and responsiveness are the keys to a successful relationship with a practitioner.

The techniques applied are determined through the consultation process and by conditions that present themselves during the massage.

The intensity of the work ranges from less physically demanding energy work, such as Reiki and relaxation techniques, to the more deep tissue and pressure point techniques such as shiatsu, Swedish, trigger point therapy, and assisted stretching. The deeper techniques are for injury rehabilitation, improved flexibility and athletic performance.

There should be no intolerable discomfort. The pressure used is to client tolerance, so speak-up if the pressure is too much. That said; there are benefits to tolerating some pain, depending on your desired outcome. If you are there for relaxation - no pain. If you are there for improved athletic performance - no pain/no gain.

Be aware; there is good pain and bad pain. Good pain is pain that is local to the pressure and might move SLOWLY. It is somewhat dull or achy. The best way to manage this type of pain is to breathe through it and let the therapist know if it becomes too much. Bad pain is sharp, shooting pain. This is never good. Stop the technique immediately. You might need treatment from a Chiropractor or physician.

Post Massage Care

Drink plenty of water. A massage can dislodge toxins and waste products from your tissues. Your body uses water as a medium by which to flush them. Your water needs depend on many factors, including your health, your activity level and your environment. A general rule is 8 to 12 - 8 ounce cups per day. Healthy adult kidneys can process fifteen liters of water a day. You will not suffer from water intoxication if you drink a lot of water, provided that you drink it over a span of time. Do not drink large amounts all at once.

Activity level after a massage, again, depends on outcome goals. Plan to relax for the remainder of the day.

Do not plan a competitive athletic event for three days after a deep tissue massage. If that is unavoidable, it is recommended that you do a light workout (30 to 50% of normal workout intensity) before the event. This prevents the muscles and joints from being too lax and vulnerable to injury.

If you have residual pain from a deep massage, use ice to reduce any sore spots.

If you have a headache, it is most likely because you did not drink enough water.

Following these guidelines goes a long way to insuring a positive first massage experience. These processes and technique are employed in our clinic with much success.

4 Steps to Correcting Mallet Toe


Mallet toe and hammertoes are deformities of the joints. They form for a number of reasons. Identifying that reason and treating it is usually necessary to prevent the deformity from progressing further. Reversing it may require surgical correction, if it is not caught in time.

So, if you are starting to see changes in the joints of your toes, it is time to take action. Here's what to do.

Identify the Reason

What is causing the joint change? Rheumatoid and osteoarthritis cause damage to the joints. Although, past treatments for arthritis focused on pain management, other courses of action are now recommended, included regular physical activity and weight bearing exercises.

The length of your toes and your general foot health could be the cause of the joint changes. For example, if your second toe is longer than your big toe, it could be in a constantly cramped position due to poor fitting shoes, a tight toe-box or hard shoe materials. Bunions and plantar warts can also cause the toe's tendon to shorten and eventually produce the deformity.

High arches and flat feet can eventually cause the joint changes. Wearing shoes with no arch support can eventually cause the problems, too.

In addition to arthritis, other diseases can cause mallet toe and hammertoes. Or, at least, the condition is more common in people that have diseases like diabetes. A stroke can be a contributing factor. Joint changes are classic symptoms of Charcot-Marie-Tooth disease.

Correct the Cause

If none of the above causes seem to apply, then you are not wearing friendly shoes. You need to face that fact, before you proceed. Our feet should be cared for in the same way that we care for the rest of our bodies. Fashion designers have made it difficult for people, especially women, to stand and walk in comfort.

If the cause of your mallet toe is an underlying disease, a trip to the podiatrist is in order. Unlike your regular family doctor, a podiatrist specializes in treating foot problems. Diabetics should visit a podiatrist regularly.

For high arches, custom orthotics is the solution. Orthotic inserts are also helpful for people that suffer from Charcot foot.

For flat feet, arch supports are the answer. In fact, anyone will benefit from good arch support.

Exercise Your Toes

Physical therapy for the toes might seem like an unusual idea, but some exercises will help to stretch the tendons and strengthen the joints. For example, curling up a newspaper with your toes or gently stretching them manually may help reverse mallet toe.

Use Straighteners and Separators

Straighteners, separators and caps combined with practical shoes that have a large deep toe box are helpful for anyone, regardless of the cause of the problem. Even if they cannot reverse the condition completely, they will help prevent it from worsening.

When you choose shoes, make sure that there is plenty of room to wiggle your toes. If not, then the shoes are too tight. Surgery is sometimes necessary to correct mallet toe. It's best to avoid that option, if at all possible.

Finding Out What Treatment a Massage Client Really Wants


What Does The Client Want?

This is the simplest and most difficult part of building your client base - and also the most important thing to remember.

It is a very simple concept, but it can be very difficult to achieve and if you don't the clients won't come back or give you referrals.

So what is it?

Give the client the treatment they want.

That's it - I said it was simple.

Or is it?

Let's take an example of a client booking a one hour Massage appointment. They make the booking, turn up on time and you go through your intake process to ensure no contra-indications etc and you then give them the massage. At the end they leave satisfied having had the treatment they booked - a one hour massage.

Did they get the treatment they really wanted?

Each client is unique; they each have a different body with different tensions, areas of stress and needs. Sometimes a client has a specific physical problem they want addressing - knotted shoulders for example, other times they just want to have an hour dedicated to them in a nice pampering massage to send them off to sleep.

Which of these did the client who booked the one hour treatment want?

The difference between a treatment that is going to help ease the tension in some tight shoulders and one that is going to send a client drifting off to sleep in the clouds is immense, and if you don't give the client the right one they aren't going to be very satisfied with the treatment.

And if they aren't satisfied they had the treatment they wanted they aren't going to come back again.

So perhaps the key to an excellent treatment should be:

Find out what the client wants.

This is really important, I have been for so many massages over the years - from small private practices to some of the world's best Spa's in five star hotels.

Whenever I travel I like to have a massage, particularly if it is a new country or destination. From all of these treatments I have learnt something, just because you are paying a lot for a treatment in beautiful surroundings doesn't mean you are going to get a good treatment.

Most places, not all, have some sort of case history consultation. You need to check for any contra-indications before beginning any treatments, but all too often what is usually just a form is as far as the therapist goes in finding out my needs and desires for the treatment.

So here we have it, the first key to ensuring repeat clients - spend a few moments before the treatment starts asking them what they want. What type of treatment are they after.

But what does the client really want?

Ok, that's brilliant - we've asked the client what they would like from the treatment but unfortunately they have a tendency to be not very helpful. Very quickly, and I bet you've used these yourself, you will get some answers like:


"Just to relax and chill-out"

"I'm just really stressed with so much tension I need a really deep massage"


And of course the most unhelpful of them all:


"I don't know really, I just fancied a massage"


These, or similar, are the three most common answers I have found that clients give when you ask them what they want from a massage.

Our job as therapists is to understand what they really mean by their answer, to help them expand on it and to explore it. Generally they have come for a massage or treatment for a reason, they want to feel different when they leave to how they arrived.

And this is the second key to ensuring the client gets the massage they want:

How does the client want to feel when they leave?

I think this is really important, I always ask all my clients this because it makes them think about the outcome of the treatment a little more. Wording can be changed to suit, but some examples I use are:


"What will you notice that is different about yourself or in your body when you leave?"

"Do you want to feel different when you leave? If so how - describe how you would like to feel emotionally and physically"

"If you have the massage you want today, what will be different between now and when you leave?"


Like us, our clients are often lazy and don't want to think about the answers - often replying with?"I don't know" or "Relaxed." So in this situation I follow up with some more probing, similar to some of the following


"Ok, will you walk differently?"

"When you walk out of here to go to your car will I notice anything different about you or how you are walking?"

"Relaxed, how does relaxed feel for you? Can you describe it?"


You will then find that most clients will start to expand on their answers and give you a much better idea of how to plan your treatment.

At the end of the treatment you can ask them how they feel, did they get the outcome they wanted. A very powerful way of doing this is to ask them to walk around the treatment room and describe how they feel. Ask how it compares to when they arrived, what is different and do they feel they got the treatment they wanted. When they are walking they will often stand still to think about the answer, prompt them to keep walking. Through the movement of their body they will really start to notice the changes and differences.

So I Passed My Massage Boards: Now What?


It was not too long ago that I felt that exciting rush of anticipation as I realized I was about to graduate from massage school and prepare for the National Boards. I studied for a month, practicing massage techniques and scrutinizing my anatomy books. I, like many others, passed with flying colors! Then I rented an office. And waited. What now? I, unfortunately, was amongst the ranks of those who felt that if they constantly strived to perfect their craft, learn more, and become the best therapist possible with the education and experience accessible to me, my practice would become magically full! I was gravely disappointed that this did not happen. I started to think I had thrown ten thousand dollars down the drain. I started making lists of graduate programs I could apply to to go into a more "secure" field. And then, an inspiration hit!

What if I could utilize this constantly growing entity known as the social web to grow my business? What if I could stop wasting my time making flyers and calling friends, running sales that were not enough to cover my basic overhead, and utilizing all the "old school" marketing methods that I had learned about? What if I could really tap into what smart marketers and online entrepreneurs seemed to understand? They were claiming to make five and even six figures using a few very simple techniques. I wanted that for my business. I spent about two years diving completely and totally into the online marketing world. It was overwhelming, confusing, and exhilarating all at once.

I learned a few really basic things that I want to share with you so that you do not have to make the mistakes I did. First of all, I learned that potential clients do not particularly care about our product or service, namely, massage therapy. Massage Cupping - A Unique Therapy


Although it has its roots in ancient Chinese medicine, massage cupping is experiencing a resurgence of popularity in today's health-conscious society. Cupping uses suction and negative pressure to drain excess fluids and toxins, loosen adhesions, lift connective tissue, and bring blood flow to stagnant skin and muscles. This technique intensifies the therapeutic effects of massage and other body treatments.

Cupping is accomplished by depressurizing glass or bamboo cups of various sizes. The therapist lights an alcohol-soaked cotton ball and places the flame briefly inside the cup; the cup is then quickly placed on the client's body. The flame removes the air from the cup, creating a vacuum. This vacuum creates negative pressure on the skin and tissue, pulling it slightly into the cup. This action creates a sensation of light pressure on the skin and has a sedating effect on the nervous system, very similar to traditional massage. A stronger suction will mimic the rolling action of a deep tissue massage. The movement may be long and draining or circular and stimulating for stubborn knots and areas of rigid tissue. The skin will turn very red with stronger cupping, indicating that the circulation has been brought to the surface. This increased local blood supply will bring nourishment to the muscles and skin and allow for toxins to be carried away via the circulatory system. The application of analgesics and essential oil blends immediately following will facilitate the absorption of the product deep into the tissue.

The sensation of massage cupping is pleasant and unique. Most clients describe a deep warmth and a tingling sensation long after the treatment has ended. Massage cupping is recommended for all conditions for which a patient might seek massage therapy (muscle tension, headaches, neuralgia, high blood pressure, anxiety, and fatigue) and is especially effective for tightness of the iliotibial (IT) band and chronic pain, tightness, and spasm of the middle to lower back. I have found this technique particularly useful for clients who are unable to withstand the discomfort sometimes associated with deep tissue massage, thereby allowing them to reap the same benefits.

Massage cupping is yet another natural and simple technique that offers many benefits. It has been used by European, Middle Eastern, and Asian cultures for thousands of years as an effective form of treatment. Whether used alone or in combination with other therapies, massage cupping offers productive and noticeable results for my clients.

Physical Therapy And Reimbursement - Where Are We Heading?


The professions of Physical Therapy and Rehabilitation have always been professions of healing, caring and educating. In the past 10 years, we have had to adapt and advance our professional therapeutic skills to include a dual specialty of therapist and reimbursement coordinator and manager. We enter the profession of therapeutic rehabilitation to heal our clients and provide education. We are now experiencing an ever-increasing demand on our time to complete extensive documentation required by multiple insurance companies.

The amount of reimbursement received for Physical Therapy services has remained largely flat over the past several years. In the same amount of time, the amount of required documentation has exponentially increased in volume as well as in complexity. The increase in documentation requirements, with a relatively flat level of reimbursement, does not keep pace with inflation. This effectively lowers the cost of reimbursement even further. These factors combine to produce lower levels of professional productivity due to extensive documentation time, and increased cost for additional office staff to support claims processing. The end result is working harder for less profit.

The National APTA Medicare Fee Schedule Calculator provides average reimbursements for CPT codes from 2000 to present. For a PT evaluation (97001) in upstate New York reimbursement rates have ranged from a low of $60.79 in 2000 to a high of $73.19 in 2005 and have continued to fall since that time. With reimbursement rates decreasing, paperwork increasing and increasing costs of malpractice, unemployment, disability, and workers compensation insurance for private practices, where do we go to improve the efficiency of our practices and get back to what we were trained to do? Treat patients!

Many organizations and data management companies have designed software programs for the end user offering electronic medical records and scheduling. Others offer evaluation forms and Billing/Claims/Account management systems. What becomes evident while researching these software systems is two things:

a. Software packages are aimed at improving the workflow within an office or office system that must then be managed by the provider. The full responsibility of the success of the system is placed back on the Physical Therapy Practice owner and/or the office staff, which must be hired to manage, support and facilitate the system.

b. Each software system is only as up to date as the date it was published or downloaded. With insurance regulations changing on a regular basis, the practice owner must again manage processes to update EOB's, CCI edits and new memorandums released by multiple insurance companies to assure complete and timely payments.

An internet based system with real time integrated billing management, claims processing, automated coding and reimbursement regulations is a necessary tool in getting the therapist back in the clinic and getting paid in a timely, efficient and effective manner. This process must be a shared, long-term relationship allowing the therapist to do what we are trained to do; treat patients and let the practice management solution do what it is designed to do:

o process claims faster than the national average

o provide quick access to AR and claims processing and management

o facilitate documentation to assure clean claims processing

o reduce the risk of audits with streamlined documentation

As Physical Therapists we must seek practice management systems which provide expert web based billing, coding and reimbursement solutions with an electronic medical record and scheduling capabilities. As Practice owners this will allow us to focus our efforts and expertise on improved professional staff productivity, decreased documentation time, reduced complexity and ambiguity which has existed in the PT billing and reimbursement process for years, and get back to doing what we do as Physical Therapists...client care and rehabilitation.

Now think about a person, who is either single or in a non affectionate 'non touching' relationship. They wake up and go to work and in every instance where they have 'touched' someone albeit accidentally, they apologise.

It is actually possible that some people in our society today go through whole days, or even months and never get 'purposely touched' in a positive, healing, way by someone.

    Physical Therapy After Breast Augmentation For a Rapid Recovery


    Enlargement of one's breasts by plastic surgery is done by placing an implant partially underneath the pectoralis major muscle. By so doing, this causes a muscular injury and begs the important question of what is the best way to recover after this surgery. Like all muscle injuries, a more rapid recovery is achieved by using physical therapy techniques.

    After cooling or icing one's breasts for the first few hours after surgery. I like to begin initiation of a Rapid Recovery program with early physical therapy. Arm range of motion exercises (stretch) should start the night of surgery. This consists of raising your arms from your side out to 90 degrees to the shoulder level. Repeat this 10 times every two hours on the day of surgery until you go to bed that nite. The day after surgery increase the angulation of the arms from the body up to 120 degrees every three hours or so. The following day you should be able to get the arms up to 180 degrees from your side. By the fourth day after surgery begin slow wind mills of 360 degrees going forward and then back. These stretching exercises should continue for the first week after surgery.

    The second type of pectoralis physical therapy is a stretch exercise uses the help of a door way. Stand in the middle of a door way with one foot in front of the other. Bend your elbows to a 90 degree angle and place your forearms on each side of the door way. Shift your weight on to your front leg, leaning forward, until you feel a stretch in your chest muscles. Hold for 15 seconds, relax and return to starting position. Repeat this manuever 10 more times. This should be done four or more times per day during the second week after surgery. This type of stretch introduces some resistance which may be felt more down at the low sternal attachment, some of which may been partially released at the time of surgery.

    Using an early initiation of a pectoralis stretching program, one can have a more rapid recovery after breast augmentation. Beginning the night of surgery and continuing for several weeks, most patients can have complete and full recovery within this time period. I have used this program for years in my Indianapolis plastic surgery practice and have found it universally effective for breast augmentation regardless of the patient's age.
