Is A Deep Tissue Massage Painful Or Grateful?


Deep tissues massages are very popular with athletes and patients with chronically painful conditions. This massage therapy uses several rote techniques with heavy pressure and instruments to massage the deeper muscles of the body. A good AOB will give you solid legal recourse should the insurance company or the patient ever try to evade payment.

2. They present a bill and collect patient portions at the time of service. They don't waive and discount co-pays and deductibles. Which is illegal without documented financial hardship.

 A good staff member handles the new patient interview with professionalism and tact and the patient is made aware of their financial responsibilities, not a minimum wage receptionist.

 All pertinent personal and insurance information is gathered at initial interview and/or first appointment.

 Services and codes are strategically chosen based on the type of insurance the patient has and the payer rules.

 Modifiers are applied to maximize billing. All staff are trained well on how to use them.

 Patients are presented with a bill with their portions clearly stated and they pay that day.

 The billing person receives the charges and codes daily.

3. They collect insurance portions within 60 days! They don't accept insurance company stall tactics such as, "we don't have record of your claim", "it's being processed", "we need more information", "it wasn't medically necessary", etc. They apply the state and federal provider rights laws and get paid fast.

 The AOB is manually sent to the insurance company payer

 Bills are generated and submitted electronically. Electronic claims are paid within 14 days whereas paper claims can take as long as 60-90 days.

 If payment is not made within 30-45 days, a tracer is sent with a notice warning of a possible complaint with the insurance commissioner.

4. They collect 90-100% of Billed Charges! They don't accept denials of any kind such as, "Untimely submission", "Not UCR", "Not Medically Necessary", "No Benefits", and "We sent the check to the patient so go after the patient", etc.

 The insurance commissioner and patient are sent a "CC" (copy) of that letter.

 If a reimbursement check is sent to the patient, a demand is made to issue another check referencing the instructions made on the AOB form.

 When a request for "more information" is requested, they charge the insurance company a medical request fee ($35) so they stop using that stall tactic with them. And much, much more...

 If a patient has an outstanding balance owed they don't use weak collection letters, bargain, or write-off the debt. They use collection letters that work and encourage the patient to do the right thing which is to pay the debt!

 They have payment plans available for their patients that are easily setup and administered.

 They make sure to charge patient coinsurance/co-pay's at the time of service each and every visit!

5. They maximize reimbursement! They don't bill every patient exactly the same way. They don't just bill ther-ex, manual therapy, ice and ems (97110, 97140, 97010, 97014) with every patient for a mere $79 reimbursement.

 They use modifiers like -59 and -22 to get paid more for those patients who require more time and energy to treat, such as the patient who c/o neck, shoulder, back, buttock and knee pain.

 They also use the modifier -52 for when services are reduced.
6. They preserve patient loyalty They don't allow insurance companies to maliciously splice the relationship between provider and patient by using derogatory language such as "Fee's are excessive for that geographic region", "Fees are Not usual, customary, or reasonable", "Services rendered were unnecessary or not professional".

 Template letters are sent to insurance companies every time they use derogatory language in the Explanation of Benefits statements to patients/providers.

 The insurance commissioner and patient are sent a "CC" (copy) of that letter.

 They collect patient coinsurance/co-pays at the time of each visit so the patient won't have to later pay a lump-sum-bill three weeks after discharge which most people can't pay and quickly come to resent.

Studies show that patients who owe you money are more likely to file a malpractice suit against you. Studies also show that patients who pay something out-of-pocket for their healthcare services each visit get better faster.


Billing Options Available

1. Contracting out to an independent medical billing service

Most of the so called "medical billing services" are stay-at-home moms who took a weekend course on "How to Make $40,000/yr Working From Home". They learn how to purchase software, collect and input data and submit claims. They're also taught how to print business cards and present themselves as a professional organization. The problem is most of these individuals have little to no experience.

PROS Cheaper and more personable. Allows you time to market and advertise your services.

CONS Lacks experience. Most likely won't know how to appeal denials or respond to stalling tactics. Most likely paying for simple data entry.

CHARACTERISTICS

o No setup fee.

o 4-10% of gross reimbursements.

o They collect patient info and billing by fax, Fed-Ex, or PC Anywhere

o Not very good about updating you on status of claims and collections

o Reports are not very good

Results typically are 40-50% of money lost by falling through the cracks and never getting appealed and collected. Most do not know how to appeal denials, file complaints with the insurance commissioner, respond to derogatory language in EOB's, train your staff on modifiers and good coding for different type of payers, or respond well to insurance company tactics on stalling and refusing payment--all the things that make a billing system great.

If you want to find a decent billing person, one who is organized and knows the basics, ask them these questions:

o Do you have any physical or occupational therapy billing accounts now?

o Can I contact them for reference?

o Can you send me a sample of 3 reports?...monthly claims submitted, monthly paid items posted, aging report on every outstanding claim.

o What type of billing software do you use? Is it HIPAA compliant?

o How will you collect the charge/patient data from me?

o Will you teach me code strategies for each payer type (ie. workers comp, blue cross, medicare, medpay, etc)?

2. Large Medical Billing Companies

The larger medical billing companies usually work with many providers and have many accounts. They typically have more experience but that is no guarantee they know how to go beyond data entry, claims submissions and payment postings either. There is not much money in it for them to appeal denied claims because it takes human resource and time to write letters, make phone calls, and submit complaints. They would much rather do the simple data entry and get their percentages from that.

PROS Reports are better. They have more experience. Allows you time to market and advertise your services.

CONS More expensive. Probably won't do all appeals, letters to insurance commissioner and patients especially if you are a small account (less than $10,000 per month).

CHARACTERISTICS

o Setup fee

o 8-15% of gross reimbursements.

o They collect patient info and billing by website log-in, fax, Fed-Ex, or PC Anywhere

o Not very personable

Results typically are 30% of money lost by falling through the cracks and never getting appealed and collected. Most will not file complaints with the insurance commissioner or respond to derogatory language in EOB's.

If you want to find a good billing company, one that appeals denials, files complaints with the insurance commissioner, provides detailed reports of claims submitted monthly, claims paid monthly, and aging reports with 30-60-90-120 day statuses then make sure to screen them well. Ask the following questions:

o Do you have any physical or occupational therapy billing accounts now?

o Can I contact them for reference?

o Can you send me a sample of 3 reports?...monthly claims submitted, monthly paid items posted, aging report on every outstanding claim.

o How will you collect the billing/patient data?

o Will you teach me code strategies for each payer type (ie. workers comp, blue cross, medicare, medpay, etc)?

o Do you appeal denials?

o Can I see sample appeal letters that you use?

Prices are always negotiable with outside billing companies and independents but be ready to pay if you want them to do everything listed above.

3. In-house billing where an employee does the billing

I recommend doing billing in-house with an employee after a year of solid marketing, advertising, and promoting your practice. Most owners do not have the time necessary to do both adequately (as well as treat patients). If you are considering hiring an employee to do the billing be prepared to learn the in's and out's first. Even if the employee boasts about knowing billing. It's a good idea to learn it yourself, setup the system, and work closely with the employee until they demonstrate competency.



No one will go after the money owed to you and look out for the welfare of your business like you.

PROS More control over the system. Better collection rates. If monthly billing is more than $20,000/month you will save money by using an employee versus an outside service. They can also assume other admin tasks.

CONS Takes time to learn the system and set it up.

CHARACTERISTICS

Move your fingers to the top of your head at the front of your hairline. Massage the skull in small circles with strong but not stiff hands. Next move to the ears circle around the ears then focus on the earlobes and ear itself. Follow the underneath part of your jaw line end with pressing and squeezing your thyroid and parathyroid gland underneath your Adams apple.

Rotator Cuff Physical Therapy Exercises and Rehabilitation


Rotator cuff physical therapy exercises are a key component for its rehabilitation. It's typically used first to try to avoid a surgical procedure since most shoulder injuries tend to respond to these workout routines. However, severe shoulder tears and acute shoulder impingement usually require a surgical procedure and a period of recovery before starting physical therapy.

After the rotator cuff has been injured, it's important to give the shoulder time to rest until the swelling and aching has gone away. Any tenderness or pain you may experience with certain movements is often a sign of a new injury to the muscle tissue or tendons. It may be necessary to use a sling to rest the shoulder correctly, avoiding driving, and also changing your daily routine to avoid painful activities. In addition to resting, non-steroidal anti-inflammatory drugs can be taken to reduce the swelling. Swelling of your muscles and tendons is what causes most of the aching in shoulder injuries. Also reducing the swelling is crucial for a speedy recovery.

As soon as your swelling and pain are reduced, it's important to start doing exercises that target the shoulder muscle group and rotator cuff specifically to strengthen and rehabilitate. These physical therapy exercises are not common shoulder weight lifting exercises that are used to build muscle, since in those the rotator cuff muscle isn't involved.

Rotator cuff physical therapy exercises typically use very little weight or no weight at all. The body's natural resistance is normally enough to start getting the desired results. The weight of the arm will work just fine. After the muscles get better, it will be necessary to move on to small weights. Before lifting, it is imperative to begin with light stretches to prevent any further damage to the injured muscle tissue. Then move to exercises that stabilize the muscle, followed by a workout routine to strengthen it. It's amazing how fast the shoulder can heal and be back to normal after the rotator cuff muscle has had a chance to start developing.

The Basics About the National Physical Therapy Exam


The National Physical Therapy Exam (or NPTE) is the measuring stick by which all new physical therapy graduates are measured. To get into PT school, you had to overcome many course exams, and most schools require the GRE to even apply, but the NPTE is in a league of its own.

As of 2012, there are 24,848 students enrolled in 211 PT programs throughout the USA according to the Commission on Accreditation in Physical Therapy Education (CAPTE). Each of these students must graduate from a CAPTE accredited school and take and pass the NPTE in order to begin their career as an entry-level Doctor of Physical Therapy.

The NPTE is an exam that is designed to not just test the basic recall memory of PT students. From personal experience, I can tell you that the questions on the NPTE really stretch your skills of clinical reasoning and putting multiple subject materials into on problem. The entire exam is comprised of multiple choice questions that can trick even the most seasoned veterans.

Who Designs the NPTE?

The Federation of State Boards of Physical Therapy (FSBPT) creates and maintains the test in a fashion that is constantly updated and improved. The exam's purpose is to evaluate each potential clinician to make sure they have the basic skills required to practice effectively and safely. With an ever-growing body of evidence, it is increasingly important for the test to reflect current practices in the field of physical therapy.

How is the Test Administered?

The NPTE is administered through the chain of Prometric testing centers throughout the country. Each candidate must first obtain an Authorization to Test (ATT) letter that verifies that a student has completed the appropriate PT training. This registration can be performed at the FSBPT website.

How do I Register?

All registration must be completed through the FSBPTwebsite. The description can be a little confusing, so here goes the simplified version:

1- Obtain a licensure application from your state board of licensing. This link will take you to a page listing all the contact information of each state board. There is typically an application fee and a licensure fee (total approx. $100 depending on the state)

2- Officially register for the NPTE on the Federation of State Boards of Physical Therapy website

a. Fees as of 11/2012 are $370 for both the PT/PTA exams

b. Some states have fees for the Jurisprudence Exam, but most are free and "open-book" as a part of the state licensure application.

c. You must provide accurate information about who you are and where you went to school.

3- The state will then approve your eligibility for the exam and submit it to the FSBPT.

4- FSBPT will then send you an Authorization to Test (ATT) letter which you will use to actually schedule a testing date with a Prometric testing center to sit for the exam.

5- The NPTE is now using FIXED-DATE testing, meaning that you can only take the test on 4 possible dates.

6- Study Hard! If all goes well, and you pass with flying colors, FSBPT will transmit the score directly to the state where you applied. If you pass, the state will then issue you a license number and send you a physical copy within a couple of weeks. To find out faster, I called my state authority directly and was given my score.

What is a Passing Score?

This is probably the hardest question to answer. The short answer is 600/800.

The long answer is this:

The exam has 250 questions on it, but only 200 are scored. They add 50 extra questions to "test" their testability to make sure they are not too hard or too easy. When you are taking the test, THERE IS NO WAY to distinguish these unscored items from the scored questions. Therefore, answer every question like it counts!

Each question is then scaled into the perfect 800 score. The FSBPT then tweaks the passing score of 600 up or down to give some leeway for a test that is "too easy," or "too hard."

The bottom line is that you have no way of knowing if your test is "easy" or "hard," so I would recommend not worrying about the details. Just do your best and study all of the available material. Check for the current pass rates published by FSPBT.

3 Tips For Your Injured Shoulder Before Doing Rotator Cuff Physical Therapy Exercises


- Support your injured arm during your normal daily activities.

Your arm should be propped up when driving your car, while on your computer typing, or while sitting on a chair or couch at home. The arm is pulled down by gravity when it's not supported and this causes excess stress on the muscle of the rotator cuff. A nice pillow or cushion works best. If you can keep your arm supported most of the day, and prevent the natural effects of gravity, your pain and swelling in your shoulder should be reduced. It may be hard at first to keep it supported, but this will create a significant positive difference

- Put ice on the top of your shoulder every day.

A bag of peas or carrots that has been frozen works very well to ice the shoulder. The bags easily conform to the shoulder's shape. As stated before, be sure the arm is supported while icing down. Leave the frozen bag on for fifteen to twenty minutes at a time. If your shoulder is still in pain, it's possible to ice it down more than once a day. People often ice their shoulder before going to bed since it makes it easier to sleep. It's not uncommon to want to use a heating pad for your sore shoulder. It may feel good, but heat doesn't do anything to reduce the swelling, unlike ice.

- Support your hurt arm with a pillow at night while you sleep.

Putting your sore arm up on a pillow while you sleep will help reduce stress and pulling on the rotator cuff muscle. This will help to position the rotator cuff muscle in a good spot and will make it easier to sleep. If possible, sleep on the side of your good shoulder. If you sleep on your bad side, it will put pressure on the bursa causing pain and making it difficult to sleep. Also, don't sleep on your stomach or with your arms positioned over your head. This will cause unwanted stress and pain in the shoulder.

Treating Your Thumbs Like Fine China


If you want a long career in massage, it's important to treat your thumbs like fine china.

Can you remember the first time your thumbs cried for mercy while massaging? Could it have been when you gave your first neck and shoulder massage? It probably lasted all of about two minutes before your hands, in particular your thumbs, started screaming, right? What are the last words you usually hear right before the neck and shoulder massage you're receiving from your non-massage therapist husband, wife or significant other abruptly ends?

"Ow! My thumbs!"

When I teach hands-on massage to massage therapists in training, the first thing I notice is the predominant use of their thumbs. It's so painful for me just to watch the few seconds I allow it to go on before I school them. As humans, it's a natural instinct to want to use our thumbs. After all, we've been given the wonderful gift of opposable thumbs, it seems a shame not to use them at every opportunity. Especially when we find a big juicy knot in the back that just begs for a monster thumb to dig into it. Unfortunately, for massage therapists, it's not the best idea ~ not if you're hoping for longevity in the bodywork industry.

I like to refer to my thumbs, and my students' thumbs as "Fine China." The first time I told that to a class, they looked at me as if I was a nut. Well, despite the fact that I am kind of nuts, part of the reason, I'm sure, is that about half of the class didn't know what fine china was, let alone what it had to do with their thumbs. I explained that fine china is very delicate, often expensive, porcelain dinnerware that can be easily broken; and, it's only brought out for special occasions. Of course, certain porcelains can be quite rugged, but I was trying to instill a vision in their heads of something delicate and rare that needed to be protected from damage.

Being new in the game, they didn't believe the amount of damage ~ sometimes permanent ~ massaging with heavy thumb use for three to six hours in a row, several days in a row could do. This is de rigeur behavior of newbies (until, that is we start our massage trades when they think they're going to die halfway through their first hour-long massage). Every new class was the same.

"Alright, I'd say," as I set my stopwatch. "Let's have a little demonstration. When I say go, I want you to tap your forefingers and your thumbs together, like this ~ tap tap tap ~ until I tell you to stop. Ready? Set... Go!"

And so the tapping would begin. A symphony of 25 to 30 pairs of thumbs and fingers ~ tap tap tap tap tap ~ two taps per second. Ah, so many students started out quite confident, almost mocking me in their tapping.

"See?" they'd smirk. Tap tap tap... "I can do this ALL day."

Tap tap tap... One minute down... Tap tap tap... two minutes, then three... Wait!

"Is that grimacing I see?" I'd ask smugly. "Keep tapping! Keep up that pace! Don't stop until I say!" I'd command like a Drill Sergeant in Basic Training.

I have to say, I almost enjoyed proving my point, though I knew the pain they were beginning to feel. I wanted it to hurt, just enough so they'd get it. Finally, at four minutes I'd stop them and they would breathe a collective sigh of relief as they'd begin rubbing their aching thumbs.

It's unbelievable that four minutes of simply tapping two digits together can create searing pain in the thumb joint, muscles and tendons, but it can.

In a poll taken by terrarosa.com.au in 2008, thumb injuries accounted for between 65 and 83% of the 64 massage therapists interviewed.

I find that outrageous, and sad.

My first year of working as a full-time massage therapist in a busy spa, I worked injured. The whole year. I endured horrible pain in my hand, due to "Hook of Hamate" that began in massage school, and was constantly seeking some relief from my colleagues between sessions. My injury was due in part to the fact that we were so busy, partly because some of the rooms were too small to employ proper body mechanics, and mostly because I was not properly cautioned in my training against overusing my thumbs. I massage with wanton abandon, thinking I was invincible. As a result, I suffered needlessly while I learned a valuable lesson.

Thankfully, I was able to make some key changes to my work life that allowed my hand to heal; and of course, I really began working on my body mechanics. It helped to work in a room that was actually big enough to do massage, but that's another story for another day.

Where I taught, my students were going to end up with a $12,500 student loan balance upon graduation. I knew, based on my own experience, if I didn't find a way to drill into their heads just how delicate their hands, but especially their thumbs were, they would never manage to pay those loans off as massage therapists, because they'd most likely burn through their thumbs and/or hands in about two years.

I taught them that their hands were their money makers and they had to be treated like the super stars they were.

They had to be pampered and protected at all costs.

Prior to beginning trades we'd warm up by shaking our hands, singing "Shake your money makers!" It was rousing good fun, but it also taught them the importance of warming up prior to doing any bodywork. The invigorating, blood pulsing life evident in their ends at the end of the warm up reminded them what the focus of the whole exercise was.

In the midst of a massage trade where the class, with partners, would trade massages, I'd break the silence as I'd thrust my thumbs into the air, pointing to the heavens.

"What are these?" I'd bark in my finest Drill Sergeant voice.

"FINE CHINA!" chorused the students, massagers and the groggy massaged alike.

"When do we use them?"

"ONLY ON SPECIAL OCCASIONS!"

"Carry on."

Every massage class I've ever taught, I've mercilessly drilled the importance of protecting the thumbs into my students' heads, over and over and over again, so that they never forget what brings home the bacon, so-to-speak.

I was reminded of the importance of thumb consciousness when I saw a Tweet recently from a massage therapist mentioning how her thumb was healing and she could "probably" return to work in a week, or something like that. I'll admit, I don't know the particulars ~ for all I know she could have broken it playing basketball or falling off a bike ~ but the likelihood it was a job related injury is pretty high. If so, there's an example of a therapist losing work because the work she does has created undo stress on her money maker.

It's what prompted me to write this piece.

As most therapists know, thumbs are delicate. It's not too difficult to stress the muscles and tendons to the point of creating nerve damage. And, you don't even have to massage for a living to develop crippling Carpal Tunnel Syndrome. I've had several clients who've endured numerous surgeries because they "typed," "put small parts together," or "cleaned teeth" for a living. Anything you do repetitively for long periods of time can cause injury, which can sometimes end up being permanent.

I always struggled with my hands because not only was I a busy massage therapist, but I was also performing music regularly, playing guitar and bass, and in my spare time writing (typing) for hours and hours on end. Add to that home improvement projects and gardening and you've got hands that hurt like the devil most of the time.

I supposed that's what made me more conscious of hand safety, and such an advocate for hand, in particular, thumb well-being. For bodyworkers, and massage therapists specifically, your thumbs and your hands are your livelihood and shouldn't be taken for granted that they'll get you all the way to retirement. Treated right, they can take you all the way. Treat them wrong, and they'll take you nowhere.

The Benefits of Indian Head Massage


The head massage comes from the Indian technique called Champissage. The technique is based on alternative medicine principle that is associated with energy flow or chakras. It has been used for thousands of years by Ayurvedic healers. Another advantage of this technique that is done using oil with the right rubbing-in movement is that; it makes for a healthy scalp, strengthens hair follicles, improves hair texture and increases hair growth.

With regularity, head massage gives the body and mind a great sense of general well being. Everybody should experience it at least once!

Massage Therapy - Physical, Emotional, Mental, and Spiritual Qualities of Touch


Touch: it's one of our most basic needs and yet we usually don't spend much time considering it. We know there's a difference between "good touch" and "bad touch," yet these terms are often limited to the discussion of sexually inappropriate touch. When we go to a master bodyworker, we sense that there's something special about the quality of touch, but it can be hard to explain just what made it so special. It's helpful to consider four aspects of touch--the physical, emotional, mental, and spiritual--to understand what's going on when we give and receive touch.

Physically, touch can run the gamut of experience from superficial or deep, fast or slow, smooth or abrupt, etc. Touch can have various physical effects on the recipient's body. It can scratch the skin, break up scar tissue, dissipate or cause bruising, distribute oils, and activate endocrine or nervous system responses.

Emotionally, we can communicate feelings such as delight, surprise, anger, fear, and sadness through our touch. On the receiving end, emotions can arise in response to getting touched. For example, many people have experienced profound emotional releases while getting a massage.

Mentally, touch can be driven by our thoughts and beliefs. We can bring our intellectual knowledge of muscular anatomy to the way in which we touch another. We may notice our thoughts and beliefs driving how, why, or where we touch another-- such as in the "right" way to shake hands in a business setting. Just as emotions surface in response to receiving touch, so do thoughts and beliefs. We may notice judgments about our bodies, memories, or sudden insights arise when we are touched.

Spiritually, our touch can be fueled from our essential nature in ways that bring connection to our spirit and to another and emphasizes our union with all. We can bring essential qualities such as love, peace, compassion, strength, and clarity into the touch we give. Receiving touch can also connect us with our essential natures and with the ground of Being. During and after bodywork, recipients may feel expanded, peaceful, and connected. Obstructions to one's true Self may be detected or transformed.

The next time you give or receive touch, notice what's going on in each of these dimensions. Recognizing the various qualities of touch can empower you beyond the concepts of what feels "good" and "bad" to more fully meet your deepest needs in the physical, emotional, mental, and spiritual realms.

What is Manual Lymph Drainage in Massage Therapy?


Manual lymph drainage (MLD) refers to a very gentle kind of massage therapy used to get rid of excess fluid from the body and improve the overall role of the lymphatic system. This particular method of massage therapy is used to treat lymphedema, which is the blocking of lymph nodes in the upper and lower extremities.

MLD dates back to the early 1930's when it was first used on patients as a useful way to detoxify the body. Nowadays, MLD is still used to improve the internal mechanism of the lymphatic system. This kind of massage is applied gently in a direction parallel to normal lymph flow in order to unclog any blocked lymph vessels and permit lymph flow to carry waste from the body freely.

This technique is performed by a massage therapist who is certified in manual lymph drainage. The therapist applies a light form of circular massage to their clients' skin. MLD massages must be performed to bare skin and no massage oils or lotions can be used in order for the procedure to be effective. MLD massage is applied in mild strokes toward the heart. This technique will lightly stretch the walls of the lymph vessels and promote lymph drainage.

During an MLD massage, the therapist may utilize one of four very effective MLD techniques to promote lymph drainage:



  • Scoop strokes--In this technique, the massage therapist positions his palms in the face up position with the fingers outstretched such that hands are cupped and resemble scoop. The therapist then applies twisting strokes to the patient's skin to promote waste disposal.


  • Pump method--The massage therapist positions his palms face down on the patient's skin and makes ovoid strokes with his fingers and thumbs to promote lymph flow.


  • Rotary method--The therapist massages the skin in circular motions with his palms down on the skin. His wrists are used to apply and reduce the pressure of the strokes.


  • Stationary circles--These strokes are applied in a stationary position using circular motions with the use of the therapist's fingertips. This is normally applied over the neck, face, and lymph nodes.
